Form breakdown

What we're asking and why.

01

Athlete & Family Background

Why we ask: This establishes the core identity of the athlete and provides essential contact and background information. Knowing sibling information can also help with future recruiting pipelines or understanding family dynamics regarding athletic commitments.

Parent(s) / Guardian(s) Names*

Paragraph

Sibling(s) Names (and ages/grades if applicable)

02

Academic Profile

Why we ask: Academic standing is just as critical as athletic ability, especially for college recruitment. Knowing GPA, course rigor (Honors/AP), and intended major helps coaches tailor academic support and guide athletes toward appropriate college programs. Asking for a transcript right away saves time during the recruiting process.

Short Answer

Current Cumulative GPA*

Paragraph

Are you currently taking Honors, AP, or Dual Enrollment classes? (Please list them)

Optional
Short Answer

What college major or degree path do you plan to pursue?*

File Upload

Please upload a copy of your most recent academic transcript (PDF or Document).

Optional
03

Social Media & Highlight Links

Why we ask: In modern scouting and recruiting, film and social presence are non-negotiable. Having these links centralized allows coaches and recruiters to easily review game tape, evaluate character and public behavior, and tag athletes in promotional graphics or highlight reels.

04

Health & Injury Status

Why we ask: This is a critical liability and safety section. Coaches must know about pre-existing conditions or injuries before designing workouts or putting an athlete on the field or court.

Paragraph

Do you have any current injuries, aches, or physical limitations? (If none, type "None")*

Multiple Choice

Have you been cleared by a medical professional for full athletic participation?*

Options: Yes · No · Pending clearance

05

Personal & Sports Goals

Why we ask: Understanding why an athlete is playing helps you coach them better. An athlete aiming for a Division I scholarship needs a different motivational approach than an athlete playing purely for fun or high school camaraderie. It also shows you care about their life beyond the sport.

Paragraph

What are your primary goals with sports? (e.g., college recruitment, skill mastery, winning a championship)

Optional
Paragraph

What are your broader goals in life outside of sports?

Optional
06

Daily Readiness

Why we ask: This acts as a quick snapshot of the athlete's current physical and mental state. It helps trainers and coaches identify who might need a lighter load that day to prevent injury from overtraining.

Linear Scale (1–10)

Overall Energy & Readiness to Train Today

1 = Exhausted · 10 = Fully Energized
